Please register me for Loansome Doc Services to be provided by the Washington State University Libraries. I hereby agree to the following conditions of receiving this service:

I agree to pay applicable delivery charges as outlined below including any applicable copyright fees. I will be billed on a monthly basis and agree to pay charges upon receipt of invoice. Unpaid bills will result in automatic cancellation of Loansome Doc Services.

I may specify a delivery method (email as TIF or PDF, mail, FAX, pick up, etc.) for each individual request.

I may specify that any or all requests be referred to another library if the material requested is not available in the WSU Libraries collections. See list below for charges associated with referred requests.

I will provide my WSU account number in the Comments section of each Loansome Doc request. Upon registration with WSU, an account number will be supplied with an acknowledgement letter.

I may call (509) 335-9556 to cancel a request. I will have the request number ready when I call.
Charges cannot be cancelled if the request has already been filled.

I agree to pay fees for Loansome Doc service based on the following:

	Basic Fees (photocopy of an article up to 20 pages)
	If request is filled from WSU collections		$11.00
	If request is referred to and filled by another library		$21.00
	NOTE: Above charges include $2 per article invoice fee

	Additional Charges Per Request if Applicable
	Photoduplication from paper copy for more than 20 pages		$.15 per page of copy
	Photoduplication from microform for more than 10 pages		$.25 per page of copy
	International delivery (excluding British Columbia and Alberta)		$15.00
	Rush processing		$8.00 per request
	Fax delivery		$8.00 per request
	FedEx or other courier service delivery		Cost of delivery
	Copyright fees		Varies
	Rates are reviewed periodically and are subject to change.

	NOTICE: WARNING CONCERNING COPYRIGHT RESTRICTIONS
	The copyright law of the United States (Title 17, United States Code) governs the making of photocopies or other reproductions of copyrighted material.
