Procedures for Requesting Ergonomic Evaluation
- Employee reports ergonomic concerns to immediate supervisor.
- Supervisor and employee fill out a WSU Incident Report. A copy of Incident Report forwarded through normal channels to LAO, the Department Head, and the Health and Safety Working Group.
- Supervisor contacts the WSU Environmental Health and Safety Office (335-3041) to schedule an evaluation of the employee work area.
- The Environmental Health and Safety Office's written evaluation and recommendations forwarded to LAO and the Department Head by the supervisor.
- Department Head and Supervisor will act on recommendations from Environmental Health and Safety Office to modify work area or workstation.
- If Environmental Health and Safety Office recommends specific equipment, Supervisor and Department Head contact LAO to order new equipment.
- Supervisor will conduct follow-ups to ensure that ergonomic concerns have been addressed.
