Procedures for Requesting Ergonomic Evaluation

  1. Employee reports ergonomic concerns to immediate supervisor.
  2. Supervisor and employee fill out a WSU Incident Report. A copy of Incident Report forwarded through normal channels to LAO, the Department Head, and the Health and Safety Working Group.
  3. Supervisor contacts the WSU Environmental Health and Safety Office (335-3041) to schedule an evaluation of the employee work area.
  4. The Environmental Health and Safety Office's written evaluation and recommendations forwarded to LAO and the Department Head by the supervisor.
  5. Department Head and Supervisor will act on recommendations from Environmental Health and Safety Office to modify work area or workstation.
  6. If Environmental Health and Safety Office recommends specific equipment, Supervisor and Department Head contact LAO to order new equipment.
  7. Supervisor will conduct follow-ups to ensure that ergonomic concerns have been addressed.
